Physician Referral Required: We cannot see you without a valid order from your doctor.
It is the patient's responsibility to maintain an active prescription from the referring physician.
Our office will give you a reminder slip 7-10 days prior to the expiration of your current prescription to let you know that you need to get a renewal.
That reminder slip also gives you our fax number, so that your physician can fax the prescription to us. |
